Commentary (46 states and 5 U.S. dependent areas) that have had confidential name-based HIV infection reporting for a sufficient length of time (i.e., implemented in area since at least January 2007 and reported to CDC since at least June 2007) to allow for stabilization of data collection and for adjustment of the data in order to monitor trends. Variations in trends between groups are expected and may be due to differences in testing behaviors, targeted HIV testing initiatives, more streamlined surveillance practices in some jurisdictions, and possibly changes in the numbers of new HIV infections in some subgroups.

Data from the 2010 HIV Surveillance Report, published by the Centers for Disease Control and Prevention (CDC), indicate that, in the 46 states with long-established confidential name-based HIV infection reporting, the estimated percentage of persons living with diagnosed HIV infection who were aged 50 years or older increased 14.3%, from 28.6% in 2007 to 32.7% in 2009 [1]. Additionally, from 2005 through 2008, data from 37 states with longestablished confidential name-based HIV infection reporting revealed greater racial/ethnic disparities in HIV diagnoses among persons aged 50 years and older than among younger persons: estimated rates of HIV diagnoses (per 100,000 population) among older blacks/African Americans (49.2) and Hispanics/ Latinos (19.5) were 12.6 and 5.0 times, respectively, the rate among older whites (3.9); rates among younger blacks/African Americans (102.5) and Hispanics/Latinos (39.0) were 7.7 and 2.9 times, respectively, the rate among younger whites (13.3) [2]. Older persons were also more likely than younger persons to be diagnosed with HIV infection late in the course of their disease. This surveillance supplemental report complements the 2010 HIV Surveillance Report by presenting additional data on persons aged 50 years and older. In this report, the term older refers to adults aged 50 years and older. This report is intended to guide public health strategies that focus on preventing the spread of HIV among older adults in the United States and U.S. dependent areas as well as to assist with planning for HIV treatment and care needs. The term diagnosis of HIV infection refers to a diagnosis of HIV infection regardless of the stage of disease (stage 1, 2, 3 [AIDS], or unknown) at the time of diagnosis and does not necessarily reflect when the person became infected. Diagnoses of HIV infection do not represent incidence, or new infections, because not all infected persons have been tested and reported to the surveillance system. Where indicated, counts of diagnoses of HIV infection and deaths have been statistically adjusted (estimated) to correct for delays in reporting and for missing risk-factor information. REPORT FORMAT This report presents data tables that are organized into 4 sections: 1. Diagnoses of HIV infection among adults aged 50 years and older (Tables 1a–5) 2. Adults aged 50 years and older living with diagnosed HIV infection (Tables 6a–11) 3. Deaths of adults aged 50 years and older with diagnosed HIV infection (Tables 12a–14b) 4. Stage 3 (AIDS) classification at the time of HIV diagnosis among adults aged 50 years and older (Tables 15a–16b) Section 1 (Tables 1a–5) presents trends in numbers (statistically adjusted and unadjusted) and rates (adjusted) of diagnoses of HIV infection among adults aged 50 years and older. Section 2 (Tables 6a– 11) presents numbers (adjusted and unadjusted) and rates (adjusted) of adults aged 50 years and older living with diagnosed HIV infection. Section 3 (Tables 12a–14b) presents numbers (adjusted and unadjusted) and rates (adjusted) of deaths of adults aged 50 years and older with diagnosed HIV infection. Section 4 (Tables 15a–16b) presents adjusted numbers and percentages of stage 3 (AIDS) classification at the time of HIV diagnosis among adults aged 50 years and older. Data by age group and area of residence are provided in Tables 5, 9, and 10. All 50 states, the District of Columbia, and 6 U.S. dependent areas had implemented confidential name-based HIV infection reporting by April 2008. These 57 areas are repre5

100,000 population (17.8) were those for adults aged 50–54 years (Table 1a). The estimated rate of diagnosed HIV infections among adults aged 50–64 years (12.9) was 5.6 times the rate among adults aged 65 years and older (2.3) (Table 2a). • Race/ethnicity: From 2007 through 2010, the rates among older Asians, Hispanics/Latinos, and older adults of multiple races decreased (Table 1a). Among adults aged 50–64 years, the rates among blacks/African Americans and whites remained stable, while among adults aged 65 years and older the rates decreased among blacks/African Americans but increased among whites (Table 2a). In 2010, among older adults, blacks/African Americans accounted for 46% of all diagnosed HIV infection. The rate for older blacks/African Americans (41.6) was 2.7 times the rate for older Hispanics/Latinos (15.4) and 10.7 times the rate for older whites (3.9) (Table 1a). • Sex: From 2007 through 2010, the rate among males aged 65 years and older decreased, but remained stable among males aged 50–64 years and among older females. In 2010, among older adults, males accounted for 73% of all diagnosed HIV infections. The rate among older males (13.6) was 3.2 times the rate among females (4.2) (Table 1a). • Transmission category: From 2007 through 2010, the estimated annual number of diagnosed HIV infections among older males with infection attributed to male-to-male sexual contact increased. The estimated numbers of diagnosed infections among older males with infection attributed to injection drug use and those with infection attributed to male-to-male sexual contact and injection drug use decreased, while those with infection attributed to heterosexual contact remained stable. The estimated number of diagnosed infections among older females with infection attributed to injection drug use decreased, but the number of infections attributed to heterosexual contact increased. In 2010, among older males, 60% of diagnosed infections were among those with infection attributed to male-to-male sexual contact, 23% to heterosexual contact, 14% to injection drug use, and 3% to maleto-male sexual contact and injection drug use; among older females, 82% of diagnosed infections were among those with infection attributed to het-

Because these numbers have not been adjusted for reporting delays, they should be viewed as minimums. The tables in this report present data in 2 formats (where applicable). Tables in the first format— labeled “a”—exclude data from the dependent areas (American Samoa, Guam, the Northern Mariana Islands, Puerto Rico, the Republic of Palau (where indicated), and the U.S. Virgin Islands). Tables in the second format—labeled “b”—include data from the dependent areas. Many of the tables in this report also present data on older adults stratified either by two age groups (aged 50–64 years and aged 65 years and older) or by 5-year age groups (i.e., 50–54, 55– 59, 60–64, 65–69, 70–74, 75–79, 80–84, and 85 years and older). Readers are encouraged to read all titles and footnotes carefully to ensure a complete understanding of the data presented in each table.

HIGHLIGHTS OF ANALYSES The statements in this section, unless otherwise indicated, are based on estimated data. All rates are per 100,000 population. Our standard for determining trends in numbers and rates is based on an increase or a decrease of 5% or more. Exceptions to this standard were made where numbers were small or rates were based on small numbers. Diagnoses of HIV Infection among Adults Aged 50 Years and Older From 2007 through 2010, the estimated number and the estimated rate of annual diagnoses of HIV infection among adults aged 50 years and older in the 46 states with confidential name-based HIV infection reporting remained stable. In 2010, the estimated rate of diagnoses of HIV infection among older adults in the 46 states was 8.5 per 100,000 population (Table 1a). • Age group: From 2007 through 2010, the rates among adults aged 65–69, 70–74, and 75–79 years decreased, but increased among adults aged 80–84 years. The rates remained stable among adults aged 50–54, 55–59, 60–64, and 85 years and older. In 2010, among older adults, the largest percentage of all diagnoses (47 %) and the highest rate per  HIV Surveillance Supplemental Report

• Population of area of residence: At the end of 2009, the prevalence rate of older adults living with diagnosed HIV infection in metropolitan statistical areas (376.9) was 2.6 times the rate in metropolitan areas (146.3) and 4.1 times the rate in nonmetropolitan areas (91.1). The prevalence rate was highest among older blacks/African Americans residing in metropolitan statistical areas (1,400.4) (Table 11).

erosexual contact and 18% to injection drug use (Table 1a). Adults Aged 50 Years and Older Living with Diagnosed HIV Infection (Prevalence) From 2007 through 2009, the estimated number of older adults living with diagnosed HIV infection (HIV prevalence) increased in the 46 states with confidential name-based HIV infection reporting from 209,433 (rate, 240.9) at the end of 2007 to 256,259 (rate, 280.6) at the end of 2009 (Table 6a). • Age group: At the end of 2009, the largest percentage of older adults living with diagnosed HIV infection was among adults aged 50–54 years (46%), followed by those aged 55–59 years (29%). From 2007 through 2009, the largest percentage increase in the estimated prevalence rates per 100,000 population was among adults aged 80–84 years (33%), followed by adults aged 75–79 years (28%) and aged 70–74 years (26%) (Table 6a). • Race/ethnicity: At the end of 2009, the largest percentage—41%—was that among blacks/African Americans. Among the remaining racial/ethnic groups, the percentages were 40%, whites; 16%, Hispanics/Latinos; 1% each, Asians and persons of multiple races; and less than 1% each, American Indians/Alaska Natives and Native Hawaiians/ other Pacific Islanders. Among all racial/ethnic groups, the highest estimated prevalence rate of diagnosed HIV infection was that for older blacks/ African Americans: 1,225.5 per 100,000 population. The prevalence rate among older blacks/ African Americans was 2.3 times the rate for older Hispanics/Latinos (536.0) and 8.5 times the rate for older whites (144.2) (Table 6a). • Sex and transmission category: At the end of 2009, among an estimated total of 200,540 older males, 59% of infections were attributed to maleto-male sexual contact, 21% to injection drug use, 12% to heterosexual contact, and 7% to male-tomale sexual contact and injection drug use. Among an estimated total of 55,717 older females, 66% of infections were attributed to heterosexual contact and 33% to injection drug use. The estimated prevalence rates per 100,000 population were 476.3 among older males and 113.2 among older females (Table 6a).

Deaths of Adults Aged 50 Years and Older with Diagnosed HIV Infection From 2007 through 2009, in the 46 states with confidential name-based HIV infection reporting, the annual estimated number and rate of deaths of older adults with diagnosed HIV infection increased. In 2009, the estimated rate of deaths of older adults with diagnosed HIV infection was 11.4 per 100,000 population. Deaths of persons with diagnosed HIV infection may be due to any cause (i.e., may or may not be related to HIV infection) (Table 12a). • Age group: From 2007 through 2009, the rates of deaths among older adults of all age groups increased (Table 12a). In 2009, the rate of deaths of adults aged 50–64 years (15.6) with diagnosed HIV infection was 2.9 times the rate of adults aged 65 years and older (5.4) (Table 13a). • Race/ethnicity: From 2007 through 2009, the rates of deaths decreased among older Asians, but increased among older American Indians/Alaska Natives, blacks/African Americans, Hispanics/ Latinos, whites, and persons of multiple races. In 2009, the highest rate of deaths among older adults was that for blacks/African Americans (58.8). The rate of deaths among blacks/African Americans was 3.1 times the rate among Hispanics/Latinos (18.9) and 12.3 times the rate among whites (4.8) (Table 12a). • Sex and transmission category: From 2007 through 2009, the rates of deaths increased both among older males and older females. Although the percentage increase was greater among older females (32%) compared with older males (18%), the rate of deaths among older males (19.0) was 3.9 times the rate among older females (4.9). The numbers of deaths increased across all transmission categories among both older males and older females (Table 12a).

Stage 3 (AIDS) Classification at the Time of HIV Diagnosis During 2009, at the time of diagnosis of HIV infection, 41.5% of older adults were classified as stage 3 (AIDS) (Table 15a). • Age group: The percentage of older adults classified as stage 3 (AIDS) at the time of HIV diagnosis was 40.0% among adults aged 50–54 years and was higher among older age groups (Table 15a). • Race/ethnicity: The highest percentage of older adults classified as stage 3 (AIDS) at the time of HIV diagnosis was that among Hispanics/Latinos (47.6%), followed by the percentages among Asians (47.2%), Native Hawaiians/Other Pacific Islanders (44.4%), persons of multiple races (44.2%), whites (42.3%), blacks/African Americans (38.8%), and American Indians/Alaska Natives (33.3%) (Table 15a). • Sex and transmission category: Among older males, the percentage classified as stage 3 (AIDS) at the time of diagnosis was highest among those with infection attributed to heterosexual contact (44.4%). Among older females, the percentage was lowest among those with infection attributed to injection drug use (33.5%) (Table 15a).

• Section 4 (Tables 15a–16b): adjusted numbers and percentages of older adults with stage 3 (AIDS) classification at the time of HIV diagnosis For the assessment of trends in diagnoses, deaths, or prevalence, it is preferable to use statistically adjusted (estimated) data to eliminate artifacts of reporting in the surveillance system. Rates and trends based on estimated numbers less than 12 should be interpreted with caution because the estimates have relative standard errors greater than 30% and are considered unreliable.

This report includes data from case reports from 57 areas (50 states, the District of Columbia, American Samoa, Guam, the Northern Mariana Islands, Puerto Rico, the Republic of Palau, and the U.S. Virgin Islands) that had laws or regulations requiring confidential reporting by name for adults, adolescents, and children with confirmed diagnoses of HIV infection (including stage 3 [AIDS]) as of December 31, 2010. After the removal of personal identifying information, data from these reports were submitted to CDC. Although AIDS diagnoses have been reported to CDC since 1981, the implementation of HIV infection reporting has differed from state to state. All states, the District of Columbia, and 6 U.S. dependent areas had implemented such reporting by April 2008. Data on diagnoses of HIV infection should be interpreted with caution. HIV surveillance data may not be representative of all persons infected with HIV because not all infected persons have been tested. In addition, many states offer anonymous HIV testing; the results of anonymous tests are not reported to the confidential name-based HIV infection reporting systems of state and local health departments. Therefore, reports of confidential test results may not represent all persons who tested positive for HIV infection.

Areas with Mature HIV Infection Reporting Systems An area’s confidential name-based HIV infection reporting is considered mature after 4 years—long enough for the calculation of reporting-delay estimates and the determination of reliable trends [3]. To estimate the number of diagnoses of HIV infection, we used data from 51 areas that have had laws or regulations requiring confidential name-based HIV infection reporting since at least January 2007 and that have been reporting these data to CDC since at least June 2007. The 51 areas comprise 46 states (Alabama, Alaska, Arizona, Arkansas, California, Colorado, Connecticut, Delaware, Florida, Georgia, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Michigan, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, New York, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, Washington, West Virginia, Wisconsin, and Wyoming) and 5 U.S. dependent areas (American Samoa, Guam, the Northern Mariana Islands, Puerto Rico, and the U.S. Virgin Islands). For Tables 1a–4b, we used data from these 51 areas to tabulate numbers (unadjusted), estimated numbers, and estimated rates of diagnoses of HIV infection. These data were also used to tabulate the numbers and the estimated numbers and rates of older adults living with diagnosed HIV infection (Tables 6a–8b and Table 11), the numbers and the estimated numbers and rates of deaths of older adults with diagnosed HIV infection (Tables

TABULATION AND PRESENTATION OF DATA The data in this report are provisional and include information received by CDC through June 30, 2011. The data are organized into 4 sections. • Section 1 (Tables 1a–5): numbers (statistically adjusted and unadjusted) and rates (adjusted) of diagnoses of HIV infection among adults aged 50 years and older • Section 2 (Tables 6a–11b): numbers (statistically adjusted and unadjusted) and rates (adjusted) of adults aged 50 years and older living with diagnosed HIV infection • Section 3 (Tables 12a–14b): numbers (statistically adjusted and unadjusted) and rates (adjusted) of deaths of older adults with diagnosed HIV infection

• HIV infection, stage 2: No AIDS-defining condition and either CD4 count of 200–499 cells/µL or CD4 percentage of total lymphocytes of 14–28. • HIV infection, stage 3 (AIDS): CD4 count of <200 cells/µL, CD4 percentage of total lymphocytes of <, or documentation of an AIDSdefining condition. Documentation of an AIDSdefining condition supersedes a CD4 count or percentage that would not, by itself, be the basis for a stage 3 (AIDS) classification. • HIV infection, stage unknown: No information available on CD4 count or percentage and no reported information on AIDS-defining conditions. Data on persons with HIV infection, stage 3 (AIDS) include persons whose infection has ever been classified as stage 3 (AIDS). Stage 3 (AIDS) at the time of HIV diagnosis (Tables 15a–16b) was defined as stage 3 (AIDS) classification within 3 months after diagnosis of HIV infection.

Areas with Confidential Name-based HIV Infection Reporting As of April 2008, all 57 areas (50 states, the District of Columbia, and 6 U.S. dependent areas) had implemented confidential name-based HIV infection reporting. All 57 areas were included in tabulations of numbers (unadjusted) of diagnoses of HIV infection by area of residence (Tables 5, 9, and 10). Deaths Persons reported to the national HIV surveillance system are assumed alive unless their deaths have been reported to CDC. Death data (Tables 12a–14b) include all deaths of persons with diagnosed HIV infection regardless of the cause of death. Because of delays in the reporting of deaths, 3 years (2007–2009) of death data are displayed. The exclusion of data from the most recent year allowed at least 18 months for deaths to be reported to CDC. The estimated numbers and rates of deaths resulted from statistical adjustment for delays in reporting. Readers should use caution when interpreting trend data on the estimated numbers of deaths because the estimates for the most recent year are most subject to uncertainty.

Age For presentations of data on older adults living with diagnosed HIV infection (Tables 6a–11), the agegroup assignment (for example, 50–64 years) is based on the person’s age as of December 31 of the specified year. In Tables 12a–14b, which present data on deaths of persons with diagnosed HIV infection, agegroup assignment is determined by the person’s age at the time of death. For all other tables, the age-group assignment is based on the person’s age at the time of diagnosis of HIV infection.

Persons Living with Diagnosed HIV Infection (HIV Prevalence) Because of delays in the reporting of deaths, 3 years of prevalence data are displayed. The data reflect persons living with diagnosed HIV infection at the end of each year during 2007–2009. The exclusion of data for the most recent year allowed at least 18 months for deaths to be reported and for these deaths to be factored into calculations of prevalence.

Race and Ethnicity In the Federal Register for October 30, 1997 [5], the Office of Management and Budget (OMB) announced the Revisions to the Standards for the Classification of Federal Data on Race and Ethnicity. Implementation by January 1, 2003 was mandated. At a minimum, data on the following race categories should be collected: • American Indian or Alaska Native • Asian • black or African American • Native Hawaiian or other Pacific Islander • white Additionally, systems must be able to retain information when multiple race categories are reported. In

Stages of HIV Infection The 2008 revised HIV case definition for adults and adolescents was used to classify HIV infection among older adults [4]. The revised definition incorporates an HIV infection classification staging system that includes AIDS (HIV infection, stage 3). The stages of HIV infection are defined as follows: • HIV infection, stage 1: No AIDS-defining condition and either CD4 count of 500 cells/µL or CD4 percentage of total lymphocytes of 29. Transmission Categories Transmission category is the term for the classification of cases that summarizes a person’s possible HIV risk factors; the summary classification results from selecting, from the presumed hierarchical order of probability, the one risk factor most likely to have been responsible for transmission. For surveillance purposes, a diagnosis of HIV infection is counted only once in the hierarchy of transmission categories. Persons with more than one reported risk factor for HIV infection are classified in the transmission category listed first in the hierarchy. The exception is men who had sexual contact with other men and injection drug use; this group makes up a separate transmission category. Persons whose transmission category is classified as male-to-male sexual contact include men who have ever had sexual contact with other men (i.e., homosexual contact) and men who have ever had sexual contact with both men and women (i.e., bisexual contact). Persons whose transmission category is classified as heterosexual contact are persons who report heterosexual contact with a person known to have, or to be at high risk for, HIV infection (e.g., an injection drug user). Cases in persons with no reported risk factor for HIV infection listed in the hierarchy of transmission categories are classified as “no risk factor reported or identified” [8]. No identified risk factor (NIR) cases includes cases that have been followed up by local health department staff; cases in persons who risk factor information is missing because they died, declined to be interviewed, or were lost to follow-up; and cases in persons who were interviewed or for whom other follow-up information was available but for whom no risk factor was identified. Because a substantial proportion of cases of HIV infection are reported to CDC without an identified risk factor, multiple imputation is used to assign a transmission category for these cases [9]. Multiple imputation is a statistical approach in which each missing transmission category is replaced with a set of plausible values that represent the uncertainty about the true, but missing, value [10]. The plausible values are analyzed by using standard procedures, and the results from these analyses are then combined to produce the final results. In this report, multiple imputation has been used in tables displaying estimated values by transmission categories.

addition to data on race, data on 2 categories of ethnicity should be collected: • Hispanic or Latino • not Hispanic or Latino The Asian or Pacific Islander category displayed in annual surveillance reports published prior to the 2007 surveillance report was split into 2 categories: (1) Asian and (2) Native Hawaiian or other Pacific Islander. The Asian category (in tables where footnoted) includes the cases in Asians/Pacific Islanders (referred to as legacy cases) that were reported before the implementation of the new race categories in 2003 (e.g., cases of HIV infection that were diagnosed and reported to CDC before 2003 but that progressed to AIDS after 2003) and a small percentage of cases that were reported after 2003 but that were reported according to the old race category (Asian/Pacific Islander). In tables of diagnoses of HIV infection during 2007–2010, the Asian category does not include Asian/Pacific Islander cases because these cases were diagnosed after 2003 and were reported to CDC in accordance with OMB’s Revisions to the Standards for the Classification of Federal Data on Race and Ethnicity [5]. This report also presents data for persons for whom multiple race categories are specified. Persons whose race was unknown are included in the total numbers in Tables 6a–8b and 11–14b. In this report, persons categorized by race were not Hispanic or Latino. The number of persons reported in each race category may, however, include persons whose ethnicity was not reported. Metropolitan Statistical Areas In the Federal Register for December 27, 2000, the Office of Management and Budget (OMB) published revised standards for defining MSAs in federal statistical activities [6]. These standards, which provided for the identification of MSAs in the United States and Puerto Rico, replaced the 1990 standard. The adoption of the new standards was effective as of December 27, 2000. On June 6, 2003, the OMB announced new MSA definitions based on the new standards and Census 2000 data [7]. In this report, populations of area of residence are defined as areas with populations of 500,000 or more (MSAs), areas with populations of 50,000–499,999, or areas with populations of less than 50,000 (designated as “nonmetropolitan”).  HIV Surveillance Supplemental Report

Reporting Delays Reporting delays (time between diagnosis or death and the reporting of diagnosis or death to CDC) may differ among demographic and geographic categories; for some, delays in reporting have been as long as several years. The statistical adjustment of the data on diagnoses and deaths is based on estimates of reporting-delay distributions, which are calculated by using a modified semiparametric life-table statistical procedure. This procedure takes into account differences in reporting delays due to sex, race/ethnicity, and HIV transmission categories; reporting city, state, or territory; geographic region; the size of the MSA; and the type of facility where the diagnosis was made or death occurred [3].

Rates Rates per 100,000 population were calculated for the numbers of diagnoses of HIV infection, the deaths of persons with diagnosed HIV infection, and persons living with diagnosed HIV infection. At the time of this report, complete 2010 census data were not available from the U.S. Census Bureau. Therefore, the population denominators used to compute these rates for the 50 states, the District of Columbia, and Puerto Rico were based on the official postcensus estimates for 2009 from the U.S. Census Bureau [11]. The population denominators for American Samoa, Guam, the Northern Mariana Islands, The Republic of Palau, and the U.S. Virgin Islands were based on estimates and projections from the U.S. Census Bureau’s International Data Base [12]. Each rate was calculated by dividing the estimated total number of diagnoses (or deaths, or persons living with diagnosed HIV infection) for the calendar year by the population for that calendar year and then multiplying the number by 100,000. The denominators used for calculating age-, sex-, and race/ethnicity-specific rates were computed by applying the 2009 postcensus estimates for age, sex, and race/ethnicity for the 50 states and the District of Columbia [11]. The same method was used to calculate the denominators for Puerto Rico, with the exception of race/ethnicity estimates; these data are not available for Puerto Rico. For the other 5 U.S. dependent areas, estimates from the U.S. Census Bureau’s International Data Base were used for ageand sex-specific population denominators [12].
