"One in five Medicare patients are readmitted to a hospital within 30 days after discharge…nearly half of the readmissions are linked to social problems and access to community resources."

This publication is designed to address the often unexpected challenges many seniors face when returning home from a hospital stay. Failure to manage medications, make follow-up doctor's appointments, and obtain the physical assistance they need can often lead to seniors' readmission to the hospital. Planning ahead of time before a planned or unexpected hospital visit can help patients address questions that arise during the discharge process and ensure a smooth transition home.
